Instructions:

• Fill in the names of registering Dentists and Staff Members below.
• Select the registration type from the drop down list to the right of the name. The appropriate fee will automatically be assessed.
• Enter the registrants in consecutive text boxes. If you leave a blank box in between registrants (for example, you leave the Registrant 2 box blank and enter a name in the Registrant 3 box), nothing after the blank box will be processed.
• When you are done adding names, click on the "Add to Cart" button. You will be taken to PayPal's secure site. You can either pay via PayPal (if you have an account and want to use your PayPal balance) or via credit card. If you want to pay with your PayPal balance, log in. If you want to pay by credit card, click on the "continue" link near the credit card logos.
• Once you submit your payment information and finish with PayPal, you are done registering.
• Please note that if you request a partial refund (a staff member that you registered cannot attend), PayPal allows 1 refund per transaction. The refund will be credited to the same account from which you paid for the transaction (if you paid by credit card, the refund will be issued to that same credit card). For any additional refunds, you have to contact our Executive Secretary, Marlene Glickman (732-238-1255, or click on her name to send her an email) and request that a check be sent to you.
